This checklist can help parents know what to look for in a quality childcare center.
This form needs to be completed for automatic payments through our Procare Tuition Express system.
This form is used if you need the childcare team to administer any medication to your child including oral and topical treatments and prescription or over-the-counter medications.
This form must be completed within 3 months of enrollment and every 6 months for children under 2. Children between the ages of 2 and 5 must submit follow up examinations every two years.
This form is to be completed if your child has special dietary needs. Please speak to the director if you have any questions about this form.
